If I refuse a check from a clinical trial, will they still send me a 1099? (And report it like they gave me the check?)

Yes. You can’t “refuse” a check and avoid taxation. Once you had constructive receipt, in this case the power to pick up the check, you had the income.

Can’t I tell them to donate it? I’m not worried about the tax, it’s that I’m on SSDI and had no idea they treat a trial like earned income.

Are you sure the payment would be for a substantial gainful activity?

I don’t know of such things, but I’d think about that too. Perhaps you had some expenses to earn that money that would reduce the income below the guidelines?

Edit: A search for “clinical research payment” there came up with results. Some from people who seem to know. It’s just I don’t, so I can’t vouch for anyone.
